Tyler Michot Owner, PT, DPT
Dr. Tyler Michot, PT, DPT, is the founder and owner of Reform Rehab, established in 2026 with a vision of creating a patient-centered culture built on compassion, integrity, and excellence in care. A graduate of the University of St. Augustine in Austin, Texas, Dr. Michot earned his Doctorate in Physical Therapy and has dedicated his career to improving the quality of life for the geriatric population.
With a passion for helping older adults maintain independence, mobility, and confidence, he believes physical therapy should go beyond treatment and focus on restoring dignity, function, and overall well-being. Through Reform Rehab, Dr. Michot is committed to delivering personalized care while building meaningful relationships with patients, families, and the communities they serve.
Outside of the clinic, Tyler enjoys spending time with his family, staying active through basketball, and enjoying the outdoors through hunting and fishing. He is deeply rooted in his faith and values the importance of serving others both professionally and personally. His mission is to lead Reform Rehab with a culture that feels like family—where every patient is treated with compassion, respect, and purpose.
Director of Operations
Lori Wells
Lori Wells serves as the Director of Operations for Reform Rehab, where she is passionate about enhancing the lives of older adults and fostering meaningful relationships that support patients, families, and the communities they call home.
With more than 18 years of experience in healthcare and senior living, Lori has built her career around helping individuals navigate challenges, strengthening partnerships, and improving the overall care experience. Her leadership is guided by the belief that compassion, trust, and genuine human connection are the foundation of exceptional care.
At Reform Rehab, Lori is dedicated to creating an environment where patients, families, and team members feel valued, supported, and empowered. Her mission is to help build an organization that delivers innovative rehabilitation services while ensuring every individual is treated with dignity, respect, and personalized attention.
Through her commitment to service and relationship-building, Lori plays a vital role in advancing Reform Rehab’s vision of helping individuals maintain independence, improve function, and live life to the fullest.

Rose Blakely, PTA, is a dedicated Physical Therapist Assistant with 17 years of experience specializing in geriatric care and senior rehabilitation. She is passionate about helping older adults improve their strength, mobility, balance, and independence while providing compassionate, patient-centered care.
Throughout her career, Rose has built meaningful relationships with her patients and takes pride in creating a positive and encouraging therapy experience. Her commitment to helping others and her genuine compassion make her a valued part of the Reform Rehab team.
Outside of work, she enjoys spending quality time with her family and values the importance of faith, connection, and serving others within her community.
Business Operations Lead
Linda Guthrie
Linda Guthrie serves as the Business Operations Lead for Reform Rehab, where she oversees administrative operations, patient experience coordination, and organizational support functions that help ensure the seamless delivery of rehabilitation services. With extensive experience in healthcare operations and customer service, Linda is dedicated to creating efficient processes, supporting clinical teams, and enhancing the overall patient experience.
Passionate about serving others, Linda works closely with patients, families, referral partners, and healthcare providers to foster strong relationships and maintain the highest standards of service.
